On February 13, Vardan Harutyunyan, the Head of the State Revenue Committee, signed a new decree, which approves:
1) The sample form of the NGO activity report;
2) the procedure for completing the sample report of the activities of non-governmental organizations;
3) the publication of the sample form of the report on the activities of non-governmental organizations and the procedure for submitting the State Revenue Committee under the Government of the Republic of Armenia.
The order came into force on March 11.

PROCEDURE
FOR FILLING IN OF THE MODEL FORM OF ACTIVITY OF NON-GOVERNMENTAL ORGANIZATIONS
1. Paragraph1 of the Model Form of Activity of Non-Governmental Organizations (hereinafter referred to as “the Organization”) shall contain the full name of the organization, taxpayer identification number (TIN), location, state registration number, date, month, phone (including mobile), official web site and e-mail. If in the reporting year the Organization did not have an official website, the sub-item 1.7 shall indicate that.
2. Paragraph 2 of the Report shall contain the number of members of the Organization (including volunteers) as of January 1 of the reporting year, as well as the number of members (including volunteers) who have benefited from the organization’s resources during the reporting year.
3. Paragraph 3 of the Report shall contain information on the results of the programs implemented by the Organization, the name and venue of each program, the amount spent for each project during the reporting period and the status of the program (completed or in process).
4. Paragraph 4 of the Report shall contain information on the funds and property (type, value) received during the reporting year shall be filled out indicating the sources of the receipt.
5. Paragraph 5 of the report shall contain information on the total amount of funds used and the property (type, value) used in the reporting year shall be filled out.
6. Paragraph 6 of the Report shall contain information on the total amount of expenditures for statutory purposes.
7. Paragraph 7 of the Report, including paragraphs 7.1 and 7.2, shall contain information on the number of sessions of the Organization’s Board of Directors and other collegial management bodies shall be filled out. Moreover, in case of not meeting sessions in the reporting year, in paragraph 7, including paragraphs 7.1 and 7.2, “0” shall be filled out.
8. Attachments to the report:
1) the auditor’s conclusion if the Organization has been financed from public funds during the reporting period in the amount of five million AMD or more,
2) the conclusion of the inspection body, if the statute of the Organization provides for the establishment of the supervisory body.
9. The report shall be signed by the Chief Executive Officer and the Chief Accountant.
10. The report shall be deemed incomplete unless it contains any of the information listed in Annex N 1 or contains obvious contradictions.
11. The report is filled in AMD.

PROCEDURE
FOR PUBLICATION OF THE MODEL FORM OF NON-GOVERNMENTAL ORGANIZATIONS’ ACTIVITY REPORT AND SUBMITTING STATE REVENUE COMMITTEE BY THE GOVERNMENT OF THE REPUBLIC OF ARMENIA
1. The NGO report on its activities, and the auditor’s report if mandatory, required by law and the deadlines for the reports provided by the site, and to the operation of the conclusions of the report are published in the official website Azdarar.am of the Republic of Armenia (http : //www.azdarar.am/ ).
2. A non-governmental organization shall submit one copy of the report referred to in paragraph 1 of this regulation, the auditor’s conclusion and the audit authority’s opinion (if any), within 30 days of the publication of the report and the auditor’s report, to the tax authority of its registered office.
